Passport Information
In Jefferson County, application for a passport may be made in the office of the circuit clerk, Patricia Noland. The completed passport application and proof of U.S. citizenship either in the form of an old passport or a certified copy of birth certificate. You will also need two recent photographs and the required fees.
Two separate fees are required:
Age 16 and over:
$67.00 check or money order payable to the US Department of State
$30.00 cash only to the circuit clerk due at time of processing the application
Under Age 16:
$52.00 check or money order payable to the US Department of State
$30.00 cash only to the circuit clerk due at time of processing the application
The circuit clerk execution fee of $30.00 must be paid in the form of cash only. Personal checks will not be accepted. Photograph requirements are listed on the back of the application. Applications for passports are available in the office of the Circuit Clerk, Patricia Noland.
What you need to file a passport:
1. Certified or raised seal copy of your birth certificate
2. Drivers license or photo ID
3. $30.00 cash only processing fee
4. Age 16 and older - $67.00 check or money order payable to the US Department of State
5. Under the age of 16 - $52.00 check or money order payable to the US Department of State
6. Two (2) passport photos (see requirements on back of application)
7. Under the age of 14 - Both parents must be present to sign when application is processed
8. If one parent in unable to be present a notarized consent form must be signed and attached to the application.
9. If absent parent is unable to sign consent form evidence of sole legal custody of child is required.
